How it works
Tadalafil
At a glance
- The active ingredient of Cialis®, FDA approved in 2003 and nicknamed "the weekend pill" for a reason
- Effective for up to 36 hours, or available as a low daily dose so timing never enters the picture
- Works with your arousal by increasing blood flow; unaffected by meals, so no planning around dinner
- Generic, proven, and affordable from $3.33 per dose
- Prescribed by a US-licensed physician after an online health review
What it is
Tadalafil is the active ingredient in Cialis®, approved by the FDA in 2003 as the third entrant in the ED pill class and the one that changed how these medications fit into real life. Where sildenafil gives you an evening, tadalafil gives you a day and a half, and its low-dose daily version removes scheduling entirely. It has also been FDA approved since 2011 for urinary symptoms from an enlarged prostate, which it often improves as a side benefit. Ours is the same molecule as the brand-name product, prescribed after a licensed clinician reviews your health intake.
How it works
Like sildenafil, tadalafil blocks the enzyme PDE5 so that blood vessels in the penis relax and fill more easily when you are aroused. The chemistry differs in one important way: tadalafil stays active in your body far longer. A single dose covers up to 36 hours, and the daily microdose keeps a steady low level at all times, making spontaneity the default rather than the exception. As with all medications in this class, arousal is still required; it supports the response, it does not manufacture it.
What to expect
For on-demand use, take it at least 30 minutes before sex; meals do not meaningfully interfere, so no dinner logistics. For daily use, one small tablet at the same time each day, and after about a week your baseline readiness is simply there. Most men respond well, and your clinician can adjust dose or switch you between the two approaches as your life demands.
Side effects and who it's not for
Common side effects are headache, flushing, indigestion, nasal congestion, and, more characteristic of tadalafil, occasional back pain or muscle aches, usually mild and passing within a day or two. The same absolute rule applies as with all PDE5 medications: never combine with nitrates, because the interaction can drop blood pressure dangerously. Mention any heart condition, low blood pressure, or alpha-blocker prescription during your clinician review.
FAQ
Is this the same as Cialis®?
Same active ingredient, same mechanism, same doses. Cialis® is the brand; tadalafil is the generic at a fraction of the price.
On-demand or daily: which should I choose?
On-demand suits men who want the medication only when it counts. Daily suits men who want to never think about timing again, who are sexually active often, or who also want the prostate-symptom benefit. There is no wrong answer, and switching later is easy.
Tadalafil or sildenafil?
Tadalafil trades sildenafil's shorter, well-mapped window for a much longer one and a daily option. Men who value spontaneity tend to land on tadalafil; men who use it occasionally and deliberately often stay with sildenafil. Your clinician will help you choose.
The mechanics work, but the desire is low. Now what?
Then the pill is solving the wrong problem. Desire lives in hormones and brain signaling, not blood vessels. Have your testosterone checked (our TRT page explains how that works) or read about PT-141, which targets desire directly. This is exactly the kind of sorting your clinician review is for.
Does insurance cover this?
No. In our telehealth setup, insurance coverage is unfortunately not possible, so you pay directly. At $3.33 to $5.00 per dose depending on pack size, that is a footnote.
