What it is

Sildenafil is the active ingredient in Viagra®, and its story is one of the most famous accidents in medicine: developed in the early 1990s as a heart medication, it kept producing an unexpected side effect in trials, and in 1998 it became the first approved pill for erectile dysfunction. More than 25 years later it is generic, inexpensive, and backed by one of the largest bodies of real-world evidence any medication has. Ours is the same molecule as the brand-name product, prescribed after a licensed clinician reviews your health intake.

How it works

An erection is hydraulics: arousal signals tell the blood vessels in the penis to relax and fill. Sildenafil blocks an enzyme called PDE5 that cuts that process short, so when you are sexually stimulated, blood flows in more easily and stays longer. Two things follow from this mechanism. First, it only works alongside arousal; it will not cause an erection at your desk. Second, it fixes the mechanics, not the mood: if your problem is desire rather than performance, look at PT-141 or have your testosterone checked.

What to expect

Take it 30 to 60 minutes before sex, ideally not right after a heavy or fatty meal, which slows absorption. The window of effectiveness runs roughly four to six hours. Most men respond on the first or second try, and if the starting dose underperforms, your clinician can adjust it. Alcohol in moderation is fine; a lot of it works against you, chemically and otherwise.

Side effects and who it's not for

The common side effects are headache, facial flushing, stuffy nose, and indigestion, usually mild and short-lived. A small number of men notice a temporary bluish tint to their vision. The one rule that has no exceptions: never combine sildenafil with nitrates (nitroglycerin, isosorbide, or "poppers"), because the combination can drop blood pressure dangerously. Tell your clinician about any heart condition or blood pressure medication during review; that is exactly what the review is for.

FAQ

Is this the same as Viagra®?

Same active ingredient, same mechanism, same doses available. Viagra® is the brand; sildenafil is the generic. The difference is price, and it is not small.

Sildenafil or tadalafil?

Sildenafil is the on-demand classic with a four-to-six-hour window. Tadalafil lasts up to 36 hours and also comes in a low daily dose that removes timing from the equation entirely. If you plan, sildenafil serves you well; if you would rather never plan, look at tadalafil. Your clinician will help you choose, and many men try both before settling.

It works, but my interest just isn't what it was. Is this still the right product?

Maybe not, and that distinction matters. Sildenafil fixes blood flow, not desire. If the machinery works but the drive is missing, the more likely culprits are low testosterone or the desire pathways in the brain.
