What it is

Semaglutide is a GLP-1 receptor agonist: a lab-made copy of a hormone your own body produces after you eat. How it works

GLP-1 is one of the hormones your gut and brain use to regulate appetite. When you supplement it at higher levels than your body makes on its own, three things happen. Your brain's appetite centers quiet down, which many patients describe as the "food noise" finally going silent. Your stomach empties more slowly, so you feel full sooner and stay full longer. And your pancreas releases insulin more efficiently when your blood sugar rises, which supports metabolic health. You take it as a once-weekly injection with a very small needle, starting at a low dose that increases gradually over several months so your body can adjust.

What the research shows

Semaglutide is one of the most studied weight-loss medications ever brought to market. The largest study, a clinical trial called STEP 1 published in the New England Journal of Medicine, followed 1,961 adults with overweight or obesity who took 2.4 mg weekly or a placebo for 68 weeks. The semaglutide group lost 14.9% of their body weight on average, versus 2.4% on placebo. About 86% of participants lost at least 5% of their body weight, and half lost 15% or more. A separate trial of 17,604 people (SELECT) found that semaglutide reduced major cardiovascular events such as heart attack and stroke by 20% in people with overweight and existing heart disease. These trials used the FDA-approved brand-name product; compounded semaglutide itself has not been through clinical trials.

What to expect

Most people notice reduced appetite within the first weeks, with steady weight loss building over months as the dose increases. The trials above ran 15 to 17 months, and that is a realistic frame: this is a metabolic treatment, not a quick fix. Side effects and who it's not for

The most common side effects are digestive: nausea, diarrhea, constipation, and vomiting, usually mild and most noticeable when the dose steps up. Rare but serious risks include pancreatitis and gallbladder disease. Why glycine or niacinamide, and which one will I get?

Your prescription is personalized: based on your intake, your clinician prescribes semaglutide compounded with either glycine or niacinamide. Pricing is the same for both. Glycine is an amino acid your body uses to build muscle tissue and collagen and to regulate sleep, and early research suggests it may help protect lean muscle while you eat less; in the vial, it also plays a practical role in keeping the peptide formulation stable. Niacinamide is a form of vitamin B3 that your cells convert into NAD+, a molecule central to energy production, which is why it is paired with a medication that reduces how much you eat. To be clear about the evidence: these combinations themselves have not been through clinical trials. The weight loss comes from the semaglutide; the addition is a supportive ingredient chosen for you.

Why is it more expensive than some offers I see online?

What happens if I stop?
